We accept patient referrals here at LifeConnect24. If you are referring a patient who would benefit from a Lifeline Alarm, please enter their details below and we will contact them on your behalf.
During the hospital discharge process, we know that timing can be everything. That’s why we will contact the patient as soon as we receive an online referral and offer free, fast delivery on all our alarms.
The majority of our referrals come from hospitals, but we also accept referrals from friends and family where applicable.
